These templates use the same ConsumerWise method: define the issue, identify the evidence, ask for an answer and state what would put it right.
Formal complaint
Start or tighten a complaint with numbered issues, evidence and a specific outcome.
Telecom billing dispute
Challenge an exact charge, then follow up if the provider simply says the bill is correct.
Credit-file dispute
Correct inaccurate entries, default details or a dispute that has been generically “verified”.
Solicitor complaint
Complain about legal service and chase a final response after the provider’s complaint period.
Your personal data
I need my data, a correction or a data-protection complaint.
Choose the action that matches the problem. A request to exercise a right is different from a complaint about how the organisation handled your information.
Subject Access Request
Choose a broad SAR or a targeted request for a known event or record set.
Overdue SAR chaser
Chase a late response while accounting for ID, clarification and permitted extensions.
Missing SAR data challenge
Identify the missing category, why it should exist and what search needs checking.
Rectification request
Ask for an exact correction, then challenge a refusal and request restriction where appropriate.
Erasure request
Ask for deletion where the right applies and require reasons for any data that is retained.
Restriction request
Limit use of disputed data in qualifying circumstances and deal with a proposed lifting.
Data-protection complaint
Complain about a failed rights request or other misuse, sharing, security or accuracy problem.
Public information
I need recorded information from a public authority.
Use FOI for recorded information held by a public authority. If the request is mishandled or refused, move to internal review rather than simply resending the same request.
Escalation
The organisation has reached its final position, or a decision itself is now the problem.
These templates help you move the existing record to the next stage without losing control of the original issues.
Deadlock / final-response request
Get the organisation’s final position and, for telecoms, the correct route to ADR.
Ombudsman complaint
Present numbered issues, the provider’s position, evidence, impact and the remedy sought.
Decision challenge
Identify a material error or omission in a reviewable decision rather than re-arguing everything.
Build the record
I need to organise the evidence before I send anything else.
Use these as working documents. They are often more valuable than adding another page of argument to the complaint.

Build a neutral timeline linking events to evidence and issues.

Give every important record a stable reference and state what it proves.
Contradiction schedule
Put conflicting records or statements side by side and identify what could resolve them.
Failure-to-answer schedule
Map what you asked against what was actually answered and expose precise omissions.
How to use the templates
Square brackets are instructions, not wording to send.
Replace every item in [square brackets] with your own facts, or delete it if it does not apply. Where a page contains more than one version, choose the version that matches what has happened next. Do not combine every version into one enormous letter.

Disputed telephone sale
Challenge a telecom contract the provider says you accepted by phone, focusing on pre-contract information, the consent event and the transaction evidence.
Request telecom sales evidence
Ask a telecom provider for the point-of-sale and transaction records that can show what was presented, agreed, ordered and activated.
Missing contract summary
Challenge a provider that cannot show the contract summary and pre-contract information supplied before you agreed.
Sales promise vs order confirmation
Set out a contradiction between what a telecom salesperson said and what the later confirmation, contract or bill shows.
Broadband below minimum guaranteed speed
Report and evidence broadband performance below the point-of-sale minimum guarantee and ask the provider to use the correct resolution/right-to-exit process.
Activation and billing-start dispute
Challenge billing that began before a telecom service was usable by separating promised activation, technical activation, usable service and billing start.
Disputed renewal or regrade
Challenge a telecom renewal, upgrade or regrade where the provider says you accepted a new term, package, price or commitment.
Disputed telecom credit search
Challenge a telecom credit search or footprint by asking what transaction triggered it, what product was assessed and whether the credit-file record is accurate.
