Community, maternity and neonatal service complaints

Complaints about district nursing, health visiting, therapy, sexual health, maternity, neonatal and cross-organisational NHS care pathways.

Community, maternity and neonatal care rarely sits within one simple appointment. Care can pass between GP practices, community providers, ambulance services, hospital trusts and specialist centres.

The complaint should identify the whole pathway and then attribute each decision, record and handover to the organisation responsible.

Key points

  • Ask the organisations to coordinate where more than one NHS body is involved.
  • Name the lead complaint handler and preserve the complete chronology.
  • Include communication, escalation and handover failures as separate issues.
  • Serious professional or service-safety concerns may justify a parallel regulatory route.
  • Use PHSO for eligible unresolved NHS complaints after local resolution.
