Ambulance, NHS 111 and urgent-care complaints

How to complain about ambulance response, NHS 111, out-of-hours services, urgent treatment centres, triage, transport, handover and care coordination.

Urgent-care complaints often cross several organisations. NHS 111 may triage the call, an out-of-hours provider may assess it, an ambulance trust may respond and a hospital may receive the patient. The complaint must follow that journey.

Do not assume that one organisation has investigated the whole event merely because it answered its own part.

Key points

  • Immediate health needs come before complaint correspondence.
  • Identify each service, contact time, advice, dispatch decision and handover.
  • Ask the organisations to coordinate and name a lead responder.
  • Preserve call times, reference numbers and contemporaneous notes.
  • PHSO can consider eligible unresolved NHS complaints after local resolution.
